Preparing for Your Appointment
New patients are encouraged to print out a copy of our New Patient Information Form and bring a completed copy to your first appointment.
Please arrive 30 minutes prior to your scheduled appointment time to complete registration procedures. We ask that all patients bring their medical insurance card and photo ID to appointment.
At each visit, provide current insurance card(s) to ensure correct insurance filing. Any applicable co-pay payment or deductible is expected at the time of service. We encourage all patients to be familiar with their insurance policies and recommend calling your insurance company prior to your appointment if you have questions regarding your plan. We participate with most insurance companies. Please call our office for more information.
If you need to cancel or reschedule your appointment, please contact us 913-764-6262 no later than 24 hours before appointment. We look forward to scheduling your appointment.

Pre-Admission for Delivering at Olathe Medical Center
The Birth Place pre-registration form should be completed and returned to the Olathe Medical Center Birth Place Pre-admission Coordinator, or faxed to the Admissions Office at 913-791-4454. This form can be found in the folder you received at your first appointment. To make a pre-admission appointment, please contact the scheduling office at 913-791-4395. Patients are encouraged to make their appointments between the 30th and 33rd week of pregnancy.

Prenatal Imaging offers patients, along with their friends and family, a chance to view baby while capturing keepsake 3D/4D images.
